Water Baptism Application


Before you complete this application, please read all of the information about baptism on the document "The Meaning and Significance of Water Baptism".

After you have completed this application, one of our pastors will reach out to you and discuss further details.

On the day of your baptism, please wear dark pants, and we will provide you with a dark shirt. Please bring a towel and a change of clothes. You will be asked to remove your shoes, and we will help you into and out of the baptism tank.

Where would you like to be water baptized?

Please write a brief testimony/story about your faith journey, including how you came to place your faith in Jesus, and what your relationship with Him means to you today. It should be no more than 250 words in length.
